Who is eligible for the program?

Walmart associates and family members with type 1 or type 2 diabetes, who are covered under the associate's medical plan (AMP) are eligible. The plan must have OptumRx® as the pharmacy carrier which includes: Premier, Contribution, Local and Saver.

What prescription drugs qualify for the reduced copays?

The following drug classes qualify for reduced copays, assuming 90-day supplies using home delivery (all covered drugs are generics, except where otherwise noted):

  • Diabetes Medications and supplies:
    • Insulin Therapy ($25 Relion brand or $50 select brand copays). Note: Insulin Therapy Pens are excluded. 
    • Select diabetes monitoring supplies ($20 copay)
    • Non-Insulin Hypoglycemic Agents ($2 copay)
  • Blood Pressure & Cardiovascular Medications:
    • Ace Inhibitors ($2 copay)
    • Angiotensin II Receptor Blockers (ARBs) ($2 copay)
    • Lipid/Cholesterol Lowering Agents ($2 copay)
  • Depression Medications:
    • Selective Serotonin Reuptake Inhibitors (SSRIs) ($2)

Once enrolled, what should I expect?

Once in the diabetes program, you can expect to receive:

  • Up to 3 text messages a day to help you master the skills needed to successfully manage your diabetes
  • Live coaching support M-F, 9a-5p CT
  • Interactive text messaging support (as much as you want)
  • Optional personal reminders

How do I get started?

It's as easy as 1-2-3!

  1. Enroll in myAgileLife (you will need your Member ID to confirm eligibility)
  2. Opt-in to the text messaging program by replying YES to the text message you receive from 63141 (it will be sent to you after completing Step 1)
  3. Convert your 30-day prescriptions to 90-day refills, and order/fill them through Walmart Home Delivery (call 1-866-855-0740).
What information will I need to provide when I enroll in the program?

You will need to provide:

  • First & Last Name
  • Date of Birth
  • Gender
  • Member ID
  • Zip Code
  • Mobile Phone Number (for text messages)
  • Email Address

We will ask you a few questions to complete enrollment.

Do I have to stay in the program to get the medication copay discount? What if I stop partway through?

Enrollment and continued participation in the program are required. It’s a small investment of time to help you live a healthier lifestyle by managing your diabetes. If you do stop the program at any point, you will no longer receive the reduced copay benefit.

How do I convert my medications to 90-day prescriptions using Walmart Home Delivery / Mail Order?

Call Walmart Home Delivery, 1-866-855-0740. They will coordinate with your physician to obtain new prescriptions and with your retail pharmacy to convert them to mail order.

How do I find out if my Walmart Insurance Benefit is active?

Call Walmart People Services: 1-800-421-1362 or Optum Rx®: 1-844-705-7493.

Where do I look on my insurance card for my Member ID?
